What will you be doing: We are seeking an experienced, compassionate, and detail-oriented Intake Registered Nurse or Licensed Practical Nurse to join our team on a per diem basis.

This position plays a key role in ensuring patients receive timely, appropriate home health services and offers remote flexibility following the completion of onsite training.

Remote work must be done in Massachusetts.

Schedule

  • Minimum commitment of 2 shifts per month Department operational hours are 7 days per week, with day shifts between 8:00
  • AM and 5:30
  • PM
  • Compensation: Ranges from $38.00/hr. - $48.00/hr. based on licensure and experience
  • In this role, you will: Review and clinically assess incoming referrals to determine appropriateness for home health services. Ensure referrals are processed accurately and in a timely manner while maintaining compliance with agency, Medicare, Managed Care, and regulatory requirements. Collaborate with Clinical Managers to coordinate daily admissions, prioritize referrals, and manage agency capacity. Partner with Intake Coordinators to oversee the referral process from receipt through admission. Communicate with physicians, hospitals, case managers, and other referral sources to obtain clinical documentation and facilitate timely admissions. Build and maintain strong relationships with referral partners by providing education, support, and exceptional customer service. Serve as a clinical resource for the Intake team, utilizing sound clinical judgment to prioritize and triage referrals. Maintain accurate documentation within the electronic medical record (EMR).
  • What we’re looking for in you: Current Massachusetts RN or LPN license in good standing. Minimum of 3 years of nursing experience required. 1+ year of direct home health experience preferred. Medicare and Managed Care experience required. Working knowledge of home health and hospice regulations. Strong clinical assessment, critical thinking, and triage skills. Excellent communication, interpersonal, and relationship-building abilities. Proven ability to prioritize multiple referrals in a fast-paced environment. Strong organizational skills with exceptional attention to detail. Experience with electronic medical records; MatrixCare experience is preferred. Ability to work independently in a remote environment while collaborating effectively with an interdisciplinary team. Requirements

Candidates must hold a current Massachusetts RN or LPN license with at least 3 years of nursing experience and knowledge of Medicare and Managed Care. Experience with home health regulations and MatrixCare EMR is preferred.
