We are seeking a compassionate and motivated Physical Therapist Assistant (PTA) to join our rehabilitation team on a PRN basis. Under the direction of a licensed Physical Therapist, you will provide skilled therapy services that help residents improve mobility, strength, function, and independence. Key Responsibilities Provide physical therapy treatments as directed by the supervising Physical Therapist Implement individualized treatment plans based on physician orders and therapy goals Monitor resident progress and communicate updates to the Physical Therapist Document treatment sessions, progress notes, and resident outcomes accurately and timely Assist with discharge planning and caregiver education Collaborate with nursing staff and interdisciplinary team members to support positive resident outcomes Participate in care conferences and team meetings as needed Maintain a safe, clean, and organized therapy environment Ensure compliance with all state, federal, and facility regulations Help residents achieve their highest level of function and independence
