Job Description

A bit about the role... Devoted Medical was founded on the belief that if we treat each patient like we would our loved ones, we can meaningfully improve healthcare experiences and outcomes for some of America’s most vulnerable patient populations. This specific role provides social work case management support to our Specialty Care Programs: Diabetes, Hypertension, Congestive Heart Failure, and Chronic Obstructive Pulmonary Disease Management. This role will complement our interdisciplinary teams by addressing the psychosocial needs of our patients. We are on a mission to replicate best-in-class specialty care through our virtual care platform. Our interdisciplinary team combines remote monitoring, automated medication management, and high-touch disease education to deliver seamless speciality care in the comfort of our patients’ homes. We seek to better serve our patients with chronic conditions who often lack access to high-quality specialty care because of a scarcity of specialists in their community or personal barriers to traveling to seek care. Despite a mountain of evidence, less than 1 in 10 people with congestive heart failure are prescribed guideline-based medications proven to save lives and prevent hospitalization. Only 3 in 10 people with diabetes have ever received comprehensive diabetes education, and in the lucky few who have received education, it is rarely integrated with evidence-based pharmacotherapy. Specialty Care programs offer condition-specific, longitudinal clinical interventions aimed at changing the trajectory of chronic illness. This is accomplished by providing patients with the knowledge, treatment, and support required for them to become self-managers of their illness. Responsibilities for this role will include: Telephonic and video visits with patients and caregivers Establish trust and build strong relationships with our patients Interdisciplinary collaboration with teammates (including clinicians, nurses, and care coordinators) to ensure cohesive, patient-centered care for high-needs patients Conduct Social Work interventions to maximize the effectiveness of the program Psychosocial screenings with personalized care plans tailored to patients’ needs Emotional and caregiver support Navigate family/caregiver support Social determinants of health and social drivers of utilization Behavioral health navigation Long term care planning Advanced care planning Know how to balance multiple competing priorities from patients Work with family members/caregivers to help them understand the patient’s needs and how to support them Facilitate and participate in joint visits Participate in team huddles, panel reviews, interdisciplinary team rounds, and program/role growth and development Build relationships and coordinate with key local market and internal stakeholders Document all clinical interactions in the electronic health record ​Required skills and experience: Must Haves Master’s degree in social work and active license Minimum of 2 years expertise in the medical field Comfortable with technology and a fast-paced environment Nice to Haves Bilingual in English and Creole, Spanish, or Vietnamese. Salary Range

$70,000 - $80,000 / year The pay range listed for this position is the range the organization reasonably and in good faith expects to pay for this position at the time of the posting. Once the interview process begins, your talent partner will provide additional information on the compensation for the role, along with additional information on our total rewards package. The actual base salary offered will depend on a variety of factors, including the qualifications of the individual applicant for the position, years of relevant experience, specific and unique skills, level of education attained, certifications or other professional licenses held, and the location in which the applicant lives and/or from which they will be performing the job. Our Total Rewards package includes: Employer sponsored health, dental and vision plan with low or no premium Generous paid time off $100 monthly mobile or internet stipend Stock options for all employees Bonus eligibility for all roles excluding Director and above; Commission eligibility for Sales roles Parental leave program 401K program And more.... *Our total rewards package is for full time employees only. Requirements
