  • What you’ll do
  • Deliver compassionate, high-quality care to patients across the lifespan
  • Partner with providers and care teams to ensure seamless, coordinated treatment
  • Support patients and families in understanding health needs and next steps
  • Document care accurately using our EHR system
  • Participate in care huddles and team-based quality improvement initiatives
  • Serve as a key member of our Patient-Centered Health Home model

What We’re Looking For

Required

  • LPN / RMA License OR Certification, active and in good standing with the State of North Carolina
  • Graduate of an accredited program
  • Highschool graduated/GED
  • Current CPR/BLS certification
  • Valid Driver’s License

Preferred

  • One (1) year of clinical experience in a primary care, community health, or outpatient setting
  • Experience with underserved or rural populations
  • Familiarity with team-based care or PCMH/PCHH models
  • Bilingual

Requirements

Requires an active LPN or RMA license/certification in North Carolina, a high school diploma or GED, and current CPR/BLS certification. A valid driver's license is required, while clinical experience in primary care and bilingual skills are preferred.
