A bachelor's degree is required. QMHP-T required Experience working in the area of adult mental health is required.

Brief Description

The Adult Case Manager provides comprehensive case management services to individuals with mental illnesses and/or substance use disorders, with an emphasis on the SMI, SED, and ATR populations. Primary responsibilities include clinical assessments, individualized treatment and discharge planning, connection to community resources, supportive outreach, and ongoing follow-up to promote stability and continued participation in services. This position reports to the Senior Case Manager and Coordinator of Clinical Case Management Services and is supervised by the Coordinator of Clinical Case Management Services. The position is FLSA-exempt, designated as HIPAA Level Two, and subject to applicable OSHA regulations.
