About the role

The therapist will conduct comprehensive assessments and develop individualized treatment plans for long-term care patients in an outpatient setting. They are responsible for providing evidence-based interventions and collaborating with interdisciplinary teams to ensure coordinated care.

Ltc outpatient therapist

We are seeking a compassionate and skilled LTC Outpatient Therapist to join our team. The ideal candidate will provide high-quality therapeutic services to long-term care (LTC) patients in an outpatient setting, helping them improve their physical, emotional, and mental well-being.

Responsibilities

  • Conduct comprehensive assessments of patients’ physical, emotional, and cognitive needs.
  • Develop individualized treatment plans based on patient assessments and goals.
  • Provide evidence-based therapeutic interventions to LTC patients in an outpatient setting.
  • Monitor patient progress and adjust treatment plans as necessary.
  • Collaborate with interdisciplinary teams including physicians, nurses, social workers, and family members to ensure coordinated care.
  • Document patient sessions, progress notes, and treatment plans accurately and timely in compliance with regulatory standards.
  • Educate patients and their families about treatment goals, techniques, and strategies for managing conditions.
  • Maintain patient confidentiality and adhere to ethical and professional standards.
  • Participate in continuing education and professional development activities.

Qualifications

  • Master’s degree in Physical Therapy, Occupational Therapy, Speech Therapy, Psychology, Social Work, or related field.
  • Valid and current licensure or certification as required by state regulations.
  • Experience working with long-term care patients in an outpatient or similar setting preferred.
  • Strong knowledge of therapeutic techniques and interventions relevant to LTC populations.
  • Excellent communication and interpersonal skills.
  • Ability to work independently and as part of a multidisciplinary team.
  • Proficient in electronic health records (EHR) and documentation standards.
  • Compassionate, patient-centered approach to care.

Work environment

This position primarily involves outpatient clinical work with LTC patients. The therapist will work in a healthcare facility equipped to support therapeutic services and patient care.
