Complete required Face-to-Face (F2F) encounters for hospice recertification prior to the third benefit period and each subsequent benefit period.
Perform comprehensive patient assessments, evaluate disease progression, and communicate findings to the Hospice Medical Director and interdisciplinary team.
Collaborate with physicians, nurses, and other team members to support symptom management and individualized plans of care.
Educate patients, families, and caregivers regarding disease progression, symptom management, medications, and end-of-life care.
Accurately document all patient assessments and clinical findings in accordance with regulatory and hospice requirements.
Must be a qualified Nurse Practitioner. Ability to work on a PRN (as needed) basis in Decatur, IL.
