The Care Team Hospice is seeking a compassionate and experienced Nurse Practitioner to join our growing team!!. This is an excellent opportunity to make a meaningful difference by providing expert clinical assessments, symptom management, and compassionate end-of-life care for hospice patients in their homes and care facilities. As a Hospice Nurse Practitioner, you'll work alongside physicians, nurses, social workers, and other members of our interdisciplinary team to ensure patients receive exceptional, patient-centered care. A primary focus of this role is completing required Face-to-Face (F2F) hospice recertification visits while supporting patients and families through every stage of their hospice journey. We're looking for someone who enjoys working independently, values meaningful patient relationships, and is passionate about improving quality of life for patients facing serious illness. Compensation
  • $130 per completed Face-to-Face (F2F) Visit/Per Visit

Complete timely Face-to-Face (F2F) encounters for hospice recertification in accordance with Medicare guidelines.

Perform comprehensive clinical assessments, evaluate disease progression, and communicate findings to the Hospice Medical Director and interdisciplinary team.

Collaborate with physicians, nurses, and other care team members to develop and support individualized plans of care.

Educate patients, families, and caregivers regarding disease progression, symptom management, medications, and end-of-life care.

Maintain accurate and timely documentation of all patient encounters while ensuring compliance with hospice regulations and agency policies.

Qualifications

  • Current Nurse Practitioner (APRN/NP) license in the state of Indiana.
  • Graduate of an accredited Nurse Practitioner program.
  • Minimum of two years of advanced practice or clinical nursing experience; hospice, palliative care, home health, geriatrics, or primary care experience preferred.
  • Strong clinical assessment, communication, and critical thinking skills.
  • Ability to work independently while collaborating effectively with an interdisciplinary team.
  • Knowledge of hospice philosophy, symptom management, and Medicare hospice regulations preferred. Field Role Requirements
  • Valid driver's license, reliable transportation, and current automobile insurance.
  • Ability to pass a criminal background check and Motor Vehicle Record (MVR) screening.
  • Current CPR certification.
  • Current TB testing in accordance with agency policy.

Requirements

Must be a licensed Nurse Practitioner. Experience in hospice or palliative care is typically required for this role.
