We are seeking a compassionate, motivated, and skilled Physical Therapist to join our growing mobile rehabilitation team. As a Mobile Physical Therapist, you will provide one-on-one care in patients' homes, helping them regain mobility, improve function, reduce pain, and maintain their independence.

Perform comprehensive physical therapy evaluations in patients' homes.
Develop individualized, evidence-based treatment plans based on each patient's goals and functional abilities.
Provide skilled interventions to improve strength, balance, mobility, endurance, flexibility, and overall function.
Educate patients and caregivers on home exercise programs, fall prevention, body mechanics, and safe use of assistive devices.
Regularly reassess patient progress and modify treatment plans to maximize outcomes.
Complete timely, accurate documentation including evaluations, daily treatment notes, progress reports, and discharge summaries.
Collaborate with physicians and other healthcare professionals to coordinate high-quality patient care.
Build trusting relationships with patients and families through compassionate, patient-centered care.
Maintain compliance with Medicare Part B regulations, state practice acts, and agency policies.
Stay current with evidence-based practice through continuing education and professional development.
Doctor of Physical Therapy (DPT) from an accredited program. Current New Jersey Physical Therapist license in good standing. Current CPR certification. Valid driver's license and reliable transportation. Strong clinical reasoning and independent decision-making skills. Excellent communication and interpersonal skills. Experience in home health, outpatient orthopedics, geriatrics, or neurological rehabilitation is preferred, but motivated new graduates are encouraged to apply.
