About the role

The nurse is responsible for collecting patient data and collaborating with healthcare professionals to develop and implement high-quality care plans. They must evaluate patient outcomes and maintain professional knowledge to ensure world-class patient satisfaction.

Essential Job Duties

  • Participates and assists in data collection in a systematic and organized manner to provide high quality and safe outcomes.
  • Contributes collected data to the development of a plan of care in collaboration with other healthcare professionals that contributes to high quality and safe outcomes.
  • Implements a plan of care with measurable criteria consistent with world class performance levels.
  • Collaborates with other team members to evaluate the plan of care with the patient and family/support system to maintain world class levels of patient satisfaction and outcomes.
  • Contributes to a high-performance work environment by seeking educational resources that enhance and maintain current knowledge and skills.

Secondary Job Duties

1. Performs other duties as assigned or requested.

Minimum Qualifications

Education -- Graduate from an accredited school of practical nursing. Practical Nurse license from the Indiana State Board of Nursing. Some area may require a current and valid driver’s license.

Experience -- Graduate of LPN program.

Reports to

The Registered Nurse and is accountable to the Nurse Manager.

Requirements

Candidates must be a graduate of an accredited practical nursing program and hold a valid Practical Nurse license from the Indiana State Board of Nursing. A valid driver's license may be required depending on the specific area.
