Registered Nurse (RN) Case Manager Per Diem | Field-Based | Concierge Home Care Affirmed Home Care, New Jersey's premier private-pay home care agency, is seeking an experienced and compassionate Per Diem Registered Nurse (RN) Case Manager to provide exceptional in-home clinical oversight throughout Bergen, Essex, Morris, and Passaic Counties.
This field-based position is ideal for an independent, organized RN who enjoys building meaningful relationships with clients while delivering high-quality, personalized care in the comfort of their homes.
Conduct comprehensive in-home nursing assessments. Develop, implement, and oversee individualized plans of care. Obtain and maintain physician orders in compliance with New Jersey regulations. Monitor client conditions and promptly communicate significant changes. Complete accurate and timely clinical documentation. Perform aide supervisory visits and ensure quality of care. Provide clinical guidance and support to Home Health Aides and Nurses. Promote client safety, independence, comfort, and overall well-being.
Active New Jersey Registered Nurse (RN) License. Minimum of 2 years of recent RN experience in acute care, home care, or case management. Recent experience with: Ventilator and tracheostomy care G-tube/PEG tube management IV infusions Wound care Current BLS Certification. Current physical examination (within the past year). Current PPD or QuantiFERON test (within the past year), or chest X-ray completed within the past five years, if applicable. MMR immunization documentation. Valid driver's license and reliable transportation. Authorized to work in the United States. Compensation RN Assessment: $150 per visit RN Aide Supervision: $125 per visit Hourly Clinical Visits: $55 per hour Weekday On-Call: $50 per day Monday–Friday | 5:00 PM – 9:00 AM Weekend On-Call: $110 per day Saturday 8:00 AM – Monday 9:00 AM
