The Community Support Professional (CSP) provides individualized, community-based support to individuals with intellectual and developmental disabilities, empowering them to achieve greater independence and improve their quality of life. Working in the individual's home and throughout the community, the CSP assists with daily living activities, community integration, transportation, skill development, and achievement of person-centered goals outlined in each individual's service plan. This role promotes dignity, choice, and inclusion while maintaining a safe, supportive environment and ensuring services are delivered in compliance with agency standards, regulatory requirements, and documentation expectations. Supervisory Responsibilities: No supervisory responsibilities Primary Responsibilities: Complete well written and detailed daily service notes via electronic record system. Must be able to navigate the agency’s electronic records and Electronic Visit Verification (EVV) systems. Training on EVV and electronic records systems is provided. Requires the use of personal cell phone. Must be willing to work with individuals in their homes, in the community and provide transportation in accordance with service delivery. Routine duties may include but are not limited to: assisting individuals with scheduling, attending and transportation to medical appointments, household maintenance/housekeeping, meal planning and preparation, money management and budgeting tasks, promoting problem-solving and supporting social interactions and activities. Assistance with personal hygiene and support for medication management may also be required. Services are delivered in accordance with the individual’s service plan and in relation to their identified outcomes. Staff are responsible for providing services as detailed in the Individual’s Service Plan. Promotes a safe and caring environment where individuals can learn and grow while enjoying a healthy and productive lifestyle. Promotes community awareness by demonstrating responsible, courteous behavior and a positive perception of individuals with developmental disabilities. Participates in the Interdisciplinary Team Meeting process as appropriate. Interacts and speaks professionally with individuals, family members, Supports Coordinators, Guardians, Supervisors and other stakeholders. Team Member Expectations: Must be compliant with company and regulatory policies and procedures. Participate and contribute to weekly, bi-week or monthly leadership calls. Be prepared to report on applicable KPIs that best reflect the performance of your team. Cooperates with Beacon Specialized Living, the Department of Human Services (DHS) staff or any other regulatory body during any inspection, investigation or inquiry. Accept other duties as assigned.
