The Registered Nurse provides short-term acute care and implements treatment plans for individuals in mental health or substance use crises. Key duties include medication reconciliation, health assessments, and coordinating patient education within a multidisciplinary team.
The Registered Nurse is responsible for providing short-term acute care to individuals presenting to the Intensive Crisis Stabilization Center with a mental health/substance use crisis. The Registered Nurse provides direct patient care and implements treatment plans under the direction of the Nurse Manager and medical staff, following established clinical guidelines, policies, and procedures. The RN will work with a multidisciplinary team to ensure safety, promote recovery, and assist with treatment planning and discharge coordination.
Educational
REQUIREMENTS: Must be currently licensed as a Registered Nurse by the New York State Department of Education and possess either an Associate's Degree (if awarded prior to December 18, 2017), Bachelor's Degree and/or Master's Degree in Nursing from an accredited program. Bachelor's or Master's Degree preferred.
Experience
Required:
One (1) year of community mental health experience and/or at least two (2) years of mental health experience specifically working in a mental health setting.
Previous experience working with children and adolescents in a professional setting is highly preferred.
1.Responsible for the coordination of client services to include scheduling of outside clinic appointments, lab appointments, obtaining random urine analysis (UA), intramuscular (IM) and subcutaneous injection administration, when needed 2.Conducts medication reconciliation, when appropriate, or during transitions of care. 3.Conducts vitals and health assessments. 4.Ensures the completion of all medical assessments for each client, including coordinating additional services with the psychiatrist. 5.Monitors the pharmacological effects of prescribed psychotropic medications and notifies the psychiatrist of their reported effectiveness. 6.Working in conjunction with the psychiatrist and nurse practitioner, is responsible for the coordination of patient education efforts to include the benefits, risks and possible side effects of prescribed medication. This education effort is not limited to home visits but may also include the coordination of various groups to include but not limited to, medication, wellness, etc. 7.Responsible for entering the documentation in EHR. 8.Responsible for conducting shift change reports and ensure continuity of care through accurate communication and documentation. 9.Facilitate sharing of data with the individuals to whom it applies to facilitate partnered decision-making and to keep clients informed of progress 10.Complete QI Training during onboarding and participate in QI refresher training as needed. 11.Ability to cultivate a culture of inclusion for all employees that respects their individual strengths, views and experiences.
