The Nurse Practitioner is responsible for examinations and treatment of patients within their scope of practice. The Nurse Practitioner is responsible for the fulfillment of all obligations as noted by their professional association (Most cases will be Board of Medicine and the Board of Nursing). SPECIFIC JOB DUTIES Performs examinations and formulates treatment of patients including appropriate history, physical examinations laboratory and other diagnostic testing and referrals as necessary. Counsels and educates patients regarding their physical or mental health or delegates such education to a competent source. Recognizes complicated medical conditions that require referrals. Manages chronic conditions such as diabetes and high blood pressure. Prescribes medications and other treatments. Advises patients on how to prevent disease. Refers patients to community resources and agencies. Consults with their Collaborating Physician(s) as necessary. Fully documents plan of care; and executes care in a timely manner. Ensure timely completion of forms presented to them on behalf of the patient regarding the patient’s physical or mental health conditions; for the proper billing of patients and third-party payers; and forms necessary for other funding sources. Performs other work-related duties as assigned POSITION REQUIREMENTS Licensed to practice in the Commonwealth of Pennsylvania. Board Certification/eligibility by American Nurses Credentialing Center. Minimum of 2 years experience, required. English proficiency required ESSENTIAL FUNCTIONS In order to fulfill the requirements of this position, duties 1-11 are considered essential functions of the job. ORGANIZATIONAL INVOLVEMENT This position is required to participate in mandatory all staff meetings, team meetings and trainings. ---------------------------------- JOB SUMMARY The Physician Assistant is responsible to perform diagnostic and therapeutic tasks under the supervision of a Physician. The Physician Assistant is responsible for examinations and treatment of patients within their scope of practice. The Physician Assistant is responsible for the fulfillment of all obligations as noted by their professional association (Most cases will be NCCPA). SPECIFIC JOB DUTIES Performs examinations and formulates treatment of patients including appropriate history, physical examinations laboratory and other diagnostic testing and referrals as necessary. Counsels and educates patients regarding their physical or mental health or delegates such education to a competent source. Recognizes complicated medical conditions that require referrals. Manages chronic conditions such as diabetes and high blood pressure. Prescribes medications and other treatments. Advises patients on how to prevent disease. Refers patients to community resources and agencies. Consults with their Supervising Physician(s) as necessary. Completes forms presented to their on behalf of the patients regarding the patients physical or mental condition. Completes forms necessary for the proper billing of patients and third-party payers. Completes forms necessary for other funding sources. Performs other work-related duties as assigned. POSITION REQUIREMENTS Licensed to practice in the Commonwealth of Pennsylvania Board Certification by NCCPA Minimum of 2 years experience, required. Requirements

Candidates must be licensed to practice in Pennsylvania and hold Board Certification or eligibility from the American Nurses Credentialing Center. A minimum of 2 years of experience and English proficiency are required.
