Get to Know Your Team: Portneuf Medical Center (PMC) is a nationally recognized healthcare leader with 205 beds, serving as the region's Level II Trauma Center and tertiary referral hospital, and is home to the Portneuf Heart and Vascular Institute, Portneuf Cancer Center, Portneuf Medical Group, and Portneuf Air Rescue.

PMC has a Level III neonatal intensive care unit, an adult intensive care unit, nationally recognized orthopedic services, a full suite of cardiac care, Center of Excellence recognized surgeons, imaging, primary and specialty care

Responsibilities

  • Optimizes patient care by providing appropriate, safe, and cost effective pharmaceutical care including drug dispensing, IV admixture program, and providing drug information.
  • Works under the direction of the Director of Pharmacy and/or the Pharmacy Operations Manager and will oversee the work of pharmacy technicians, students, and support staff.
  • Interpret orders and transcribe or verify them accurately and efficiently in the pharmacy computer system.
  • Accurately compound and dispense pharmaceuticals.
  • Monitor drug therapy regimens for contraindications, drug-drug interactions, drug-food interactions, allergies and appropriateness of drug and dose.
  • Provide accurate and timely medication information to healthcare staff and patients.
  • Effectively and proactively interact with medical staff regarding drug therapy issue resolutions.
  • Document all clinical activities and interventions accurately and completely.
  • Sustain the hospital formulary, minimizing non-formulary procurements, utilizing therapeutic interchanges and other approved formulary protocols and/or guardrails.
  • Supervise and assist pharmacy technicians and other support staff effectively.
  • Prioritize work and lead others to comply with facility medication turn-around time policy and procedure and identify areas for improved efficiency.
  • Must serve as preceptor for pharmacy students and/or residents.
  • Support and participate in the goals of the organization’s quality improvement, risk management, and patient safety program to include but not limited to National Patient Safety Goals, Core Measures, and HCAHPS medication related initiatives.
  • Maintain appropriate controlled substance records.
  • Make appropriate evidence-based, patient-centered medication recommendations.
  • Participate in the management of medical emergencies as appropriate, including, but not limited to Code Blue.
  • Provide clinical consultation and clarification to practitioners.
  • Suggests appropriate, cost-effective therapeutic alternatives to medical staff as needed.

Qualifications

Job Requirements

  • B.S. or Pharm D. from an ACPE-accredited School of Pharmacy. Clinical pharmacy experience in a hospital setting. Completion of an accredited PGY-1 Pharmacy residency or fellowship
  • OR- 2+ years of clinical pharmacy experience with no history of residency or fellowship. Active Pharmacist license in good standing with the appropriate State Board of Pharmacy. Other applicable state requirements – i.e., sterile compounding, etc. BLS certification must be obtained within 14 days of hire or transfer into the role and prior to providing direct patient care. ALS/ACLS certification (within 90 days of hire;
