We are looking for Primary Care Nurse Practitioners (NP) and Physician Assistants (PA) to join our integrated resident care team at Eureka CHC, in Eureka, California.

Outpatient practice with excellent work-life balance Supportive, success-driven program High-performance PCMH model Join a growing APC residency alumni No hospital call Practice at : Advanced Practice Clinician (APC) Residency Program – Open Door Community Health Centers ODCHC Advanced Practice Clinician (APC) training program consists of a 12-month residency, followed by a 12-month fellowship (specifically geared toward trainees intending to remain with ODCHC in their future clinical practice).

Established in 2016, this program provides intensive training in the clinical complexity of rural family practice within both a safety-net setting and a high-performance, patient-centered medical home (PCMH) model of primary care.

Our program is specifically designed for newly graduated NPs and PAs who are committed to serving as primary care providers (PCPs) in community health centers and other safety-net settings.

We train residents in Open Door’s model of high-performance health care, which includes advanced access scheduling, planned care, the chronic care model, integrated behavioral health and primary care, and team-based collaboration—all supported by expert use of health information technology and the electronic health record (EPIC).

Eureka is historic coastal city that sits along Humboldt Bay and is known as the heart of California’s Redwood Coast.

As the largest coastal city between San Francisco and Portland, Eureka serves as the region’s center for healthcare, commerce, and cultural life while maintaining a relaxed, small-town feel.

The area offers abundant opportunities for outdoor recreation, including hiking, kayaking, and fishing.

The community is known for its vibrant arts scene, Victorian architecture, a range of public, charter, and private school options, along with a strong sense of community reflected in local events, youth sports, and year-round markets and festivals.

Education & Experience Diploma from an accredited program with credentialing through the American Nurses Association or Physician Assistant Association within the last 18 months Required Licensure & Certifications Prior to employment, candidates must possess: Current, unrestricted California license Current DEA registration Current Board Certification in specialty of practice (or board eligible if a newer graduate) Current CPR/BLS certification Valid California driver's license with an insurable driving record Comprehensive Benefits Medical, dental, and vision insurance Retirement plans Access/Production Bonus Eligible for National Health Service Corps (NHSC) Loan Repayment (up to $250,000 for qualifying providers) Compensation: Resident: $98,420.40 Fellow: $113,823.84 Make a Difference in a Community That Values You Humboldt County Guide — 101 Things To Do Take a Walk on the Coast

Requirements

Must be a newly graduated NP or PA with a diploma from an accredited program and a current California license. Required certifications include DEA registration, Board Certification (or eligibility), and CPR/BLS.
