About the role

The technician is responsible for gathering patient information, maintaining the master patient index, and managing the creation and retrieval of medical record folders. They also perform quantitative and qualitative audits for compliance and assist state officials during surveys.

Full Time position Schedule

  • Mondays- Fridays | 9:00
  • AM - 5:00
  • PM
  • Pay
  • Range: $30-35 depending on experience
  • Work Duties
  • Gather patient information by collecting information from a variety of sources; interacting with registration areas and physician’s offices
  • Maintain master patient index by completing assigned portion of daily audit trail; corrects and communicates problems according to established procedures
  • Initiate the medical record by creating and processing the patient care record folder
  • Maintain record availability and purge as required
  • Retrieve medical records as needed by various authorized professionals, following chart-out procedures; documenting reasons
  • Deliver charts to assigned areas of the health center
  • Keep health care providers informed by communicating availability or unavailability of the record
  • Maintain continuity of work operations by documenting and communicating actions, irregularities, and continuing needs
  • Work with state officials during Survey
  • Maintains the facility's clinical record system and assists in implementing record procedures;
  • Serves as a member of OBRA team;
  • Performs quantitative and qualitative audits for compliance including: patients' charts; medical records for physician telephone orders, physician monthly order, advanced directive status, lab tests and other assigned audits.
  • Conduct Medication Admin Record (MAR) Audit

Qualifications

  • High school diploma or equivalent required
  • One year or more experience; or any equivalent combination of training and experience which provides the required skills, knowledge and abilities working with medical records
  • Proficient knowledge of Point Click Care
  • Experience in a long-term care skilled nursing SNF or hospital setting and applicable long-term care and acute care medical records protocols
  • Prior experience in a retirement community with RCFE Title 22 Regulations
  • Licensed either RHIT, CPC or CCS with coding experience
  • Licensed as Registered Health Information Technician, preferred.
  • Ability to adapt to changes in work environment

Requirements

Candidates must have a high school diploma and at least one year of experience in a long-term care or hospital setting. Professional certification as an RHIT, CPC, or CCS is required or preferred.
