Have at least 2-3 years' experience in the primary care/value-based care setting, working with Medicare Advantage, Medicare, and Medicaid patient populations Must have an active and unrestrictive Texas APRN license Must have DEA license or eligible to obtain it within 2 months of date of hire Strong understanding of chronic disease management, preventative care, and quality metrics in a value-based care model Excellent documentation and patient relationship-building skills Proficient with EMR systems (eCW a plus!)
