The LPN will provide comprehensive clinical support in an internal medicine practice, including patient rooming, performing clinical procedures, and managing medical documentation. They are also responsible for coordinating patient care, handling refills, and educating patients on chronic disease management.

We are seeking a compassionate, organized, and patient-centered Licensed Practical Nurse (LPN) to join our Internal Medicine practice.

As part of our clinical team, you'll work alongside physicians, advanced practice providers, and care coordinators to deliver high-quality, evidence-based care that improves the health and well-being of our adult patients.

Responsibilities

  • include but are not limited to: Prepare and maintain examination and treatment rooms before and after patient visits, ensuring cleanliness, proper stocking of supplies, equipment readiness, and adherence to infection prevention and sterilization standards. Room patients by obtaining and documenting vital signs, medical histories, medication reconciliations, and other relevant clinical information accurately in the electronic health record (EHR). Assist providers during examinations and procedures by preparing equipment, handing instruments and supplies, and supporting patient care as needed. Perform clinical procedures within the scope of licensure, including medication administration, immunizations, injections, blood draws, point-of-care testing (e.g., PT/INR, EKGs), specimen collection and preparation for laboratory pickup, dressing changes, and suture removal, as directed by the provider. Coordinate patient care by scheduling referrals, diagnostic testing, follow-up appointments, and facilitating communication between patients, providers, specialists, and ancillary services. Respond promptly to patient phone calls, refill requests, patient portal messages, and other clinical inquiries while providing excellent customer service. Educate patients on medications, chronic disease management, preventive care, healthy lifestyle practices, and provider treatment plans to support improved health outcomes. Initiate and manage medication prior authorizations, assist with peer-to-peer review processes, and complete other clinical documentation as required. Maintain accurate, timely, and complete clinical documentation in the electronic health record while supporting quality improvement and value-based care initiatives. Safely handle, clean, sterilize, and dispose of medical equipment, instruments, specimens, and contaminated materials in accordance with OSHA, CDC, and organizational policies. Our practice is committed to value-based care, focusing on improving patient outcomes rather than simply treating illness. We emphasize preventive care, chronic disease management, care coordination, and patient engagement to help our patients live healthier lives while reducing unnecessary hospitalizations and healthcare costs. Supporting preventive screenings and wellness initiatives. Helping patients manage chronic conditions such as diabetes, hypertension, COPD, and heart disease. Promoting medication adherence and patient education. Collaborating with providers and care management teams to close quality care gaps. Contributing to quality metrics that improve patient outcomes and enhance the patient experience. Skills required: Type a minimum of 50 wpm
  • One or more years of experience in Internal Medicine, Family Medicine or Primary Care. Experience with athenaClinicals electronic medical record system is preferred but not required. Familiarity with quality measures and value-based care initiatives is preferred but not required. *A skills test may be used to provide an unbiased and validated evaluation of a candidate's ability to perform the duties listed in the job's description. Requirements

Current Licensed Practical Nurse (LPN) license in the State of Maryland. Graduate of an accredited practical nursing program. Current BLS/CPR certification. Strong communication, organizational, and critical thinking skills. Ability to work efficiently in a fast-paced clinical environment.
