Provide same-day and acute-care appointments when clinically appropriate.

Evaluate, diagnose and manage common acute and chronic medical conditions.

Manage conditions including hypertension, diabetes, hyperlipidemia, obesity, thyroid disease, asthma, COPD, common gastrointestinal conditions, musculoskeletal complaints and other adult primary-care needs.

Perform medication reconciliation and ongoing medication management.

Provide evidence-based preventive screening and immunization counseling.

Order and interpret laboratory, imaging and diagnostic testing within scope.

Develop clear treatment, monitoring and follow-up plans.

Escalate urgent or unstable conditions promptly.

Maintain clear documentation of consultation, referral and escalation.
