About the role

Provide comprehensive primary care services, including diagnosis and treatment of acute and chronic illnesses for frail elderly patients. Collaborate with an interdisciplinary team to develop and implement individualized care plans across clinic, day center, and home settings.

What is PACE?

PACE or Program of All-inclusive Care for the Elderly is a federal and state funded program that provides comprehensive, coordinated, and proactive whole-person care to help maintain and improve seniors’ health so they can live safely in their homes. HopeWest PACE provides home care, access to the day center and clinic, transportation to and from medical appointments, and activities to enhance the lives of patients and their families. This innovative program balances medical, emotional, physical, and social needs of seniors.

The Unique Responsibilities of a Physician at PACE

  • Provides wide range of Primary Care services including diagnosis and treatment of chronic and acute illness, preventative care, and urgent medical needs all in collaboration with the Chief Medical Director.
  • Conducts health assessments and evaluates patient needs in the clinic, the PACE Day Center and the patient’s home
  • Serves as a member of the robust interdisciplinary team to develop and implement the comprehensive plan of care to meet each patients’ individual needs
  • Make appropriate and timely referrals to our contracted providers out in the community

What You Bring

Licensed M.D. or D.O. in the state of Colorado

Board Certification or eligibility in Family Medicine, Internal Medicine, or another related specialty preferred.

Controlled substance license with current DEA license number or ability to obtain.

Eligible or currently possesses Medicare/Medicaid UPIN number

Three (3) years of physician experience preferred.

Experience working with older adults, medically complex populations or multiple chronic conditions preferred.

Experience in geriatrics, primary care, hospice, palliative care, home-based care, or PACE preferred.

Valid Colorado driver’s license, adequate automobile insurance and access to reliable transportation maintained throughout employment.

Strong clinical reasoning, diagnostic, and medical decision-making skills.

Knowledge of geriatric medicine, chronic disease management, preventative care and evidence-based practices.

Ability to lead and collaborate effectively

Requirements

Requires a valid M.D. or D.O. license in Colorado and a current DEA license. Preference is given to candidates with board certification in Family or Internal Medicine and three years of experience working with medically complex older adults.
