About the role

The Triage Nurse assesses patient health needs, develops care plans, and provides medical support for urgent care patients. Responsibilities include triaging, performing lab work, and collaborating with providers to initiate treatment for ambulatory diagnoses.

Become a part of our caring community The Triage Nurse (RN) assesses patients’ health problems and needs, develops and executes nursing care plans, and assists in maintenance of patient medical records.

The Triage Nurse (RN) work assignments are varied and frequently require interpretation and independent determination of the appropriate courses of action.

Under the general supervision of clinic providers, is responsible for the medical care support of urgent care patients, including triaging patients, taking medical histories, and/or performing lab work.

The Triage Nurse (RN) supports the above functions in either a clinic-based or telephonic environment.

The Triage Nurse (RN) understands department, segment, and organizational strategy and operating objectives, including their linkages to related areas.

Makes decisions regarding own work methods, occasionally in ambiguous situations, and requires minimal direction and receives guidance where needed.

Follows established guidelines/procedures.

Essential Functions: Identifies patient care requirements by establishing personal rapport with patients Establishes a compassionate environment by providing emotional, psychological, and spiritual support to patients, friends, and families.

Responsible for l evaluation of patients with ambulatory diagnoses as well as possible acute conditions.

The triage nurse will work in partnership with the center’s mid-levels and PCPs.

In consultation with the center’s mid-levels and / or PCPs the triage nurse will initiate treatment, if necessary, for common ambulatory diagnoses.

Promotes patient's independence by establishing patient care goals; teaching patient, friends, and family to understand condition, medications, and self-care skills; answering questions.

Assures quality of care by adhering to therapeutic standards; measuring health outcomes against patient care goals and standards; making or recommending necessary adjustments; following hospital and nursing division's philosophies and standards of care set by state board of nursing, state nurse practice act, and other governing agency regulations.

Resolves patient problems and needs by utilizing multidisciplinary team strategies.

Required Qualifications Must be willing and able to perform the following procedures or at least be willing to be trained to do them: 2 to 5 years of Urgent Care / Emergency Room experience Bilingual in English and Spanish Starting an IV and initiating IV fluids Give Injections Simple wound care Insert a foley catheter if appropriate Administer aerosol treatments Perform EKGs Good communication skills Works well in a team of healthcare providers. Associates Nursing Degree Nursing Diploma

Candidates must have 2 to 5 years of Urgent Care or Emergency Room experience and be bilingual in English and Spanish. An Associate Nursing Degree or Diploma is required, while a Bachelor of Science in Nursing is preferred.
