Performing face-to-face assessments and ongoing psychiatric visits with adult clients in long-term care facilities.
Maintaining current patient records to clearly document problems, treatment plans, actions, goals, and changes as they occur.
Educating and supporting caregivers and facility staff.
Managing and adjusting psychiatric medications to promote stability and well-being.

Graduate of an accredited Nurse Practitioner program.
PMHNP license required.
Experience in psychiatric, mental, and behavioral health services.
Long-term care and/or geriatric experience strongly preferred.
Outpatient and tele-psychiatry experience a plus.
Willingness to travel within assigned service area.
