The Advanced Practice Nurse conducts patient visits and collaborates with an interdisciplinary team to develop and implement plans of care. They are responsible for maintaining accurate patient records and ensuring access to appropriate support models of care.

1. Conduct visits to patients in accordance with local, state and federal regulations. 2. Communicate visit findings to designated members of the interdisciplinary team. 3. Facilitate access to appropriate support models of care (i.e., rehabilitation, on-going aggressive treatment). 4. Consult with other members of interdisciplinary team to contribute to the patient's plan of care. 5. Prepare and maintain accurate patient records, charts and documents. 6. Assist in development of clinical practice guidelines/standards in support of quality care as needed. 7. Ensure collaboration with patients, caregivers, medical directors and members of IDT.

Candidates must hold a Master's or Doctoral degree in Nursing and a valid Nurse Practitioner's license. A minimum of 1-2 years of experience in Family, General, or Geriatric practice and a valid driver's license are required.
