Position Summary

The Clinical Case Manager conducts comprehensive in‑person assessments and develops individualized care plans for members in MLTSS and D‑SNP programs. This role coordinates physical, behavioral, and social services across providers and community resources, ensuring members receive appropriate, high‑quality, and cost‑effective care. The Case Manager authorizes services, documents care activities, participates in interdisciplinary meetings, and supports team development. With flexible teleworking and meaningful community engagement, the position offers the opportunity to build trusted relationships, advocate for vulnerable populations, and apply strong clinical judgment while pursuing ongoing professional growth, including NJ Choice Certification. Join our LTSS team as a Behavioral Health Clinical Case Manager, where you’ll play a vital role in improving the lives of our members by connecting them with the care, resources, and support they need to thrive. This is a unique opportunity to blend clinical expertise with care coordination in a flexible, telework environment—while still getting out into the community to make a real impact. As a Clinical Case Manager, you will: Conduct comprehensive, face-to-face assessments for members enrolled in MLTSS and D-SNP programs Develop and implement personalized care plans that address physical, behavioral, and psychosocial needs Collaborate closely with members, families, providers, and interdisciplinary teams to coordinate care Evaluate and facilitate services such as: Adult and pediatric medical daycare Personal care assistance Nursing facility support Community-based and long-term services Advocate for members to ensure access to appropriate and high-quality care Authorize services and ensure efficient, cost-effective resource utilization Document care activities accurately in the electronic health record Participate in interdisciplinary meetings and help drive positive outcomes Support and mentor new team members as you grow in your role What Makes This Role Special Flexible telework with meaningful in-person community engagement Opportunity to build lasting relationships with members and providers Be a trusted advocate for vulnerable populations Continuous development, including NJ Choice Certification support A role where your clinical judgment and critical thinking truly matter Required Qualifications: Licensed Clinical Social Worker (LCSW), or Licensed Social Worker (LSW). 2+ years of Behavioral Health experience/expertise required. Must possess reliable transportation and be willing and able to travel up to 75% of the time. Mileage is reimbursed per our company expense reimbursement policy. Successful completion of the NJ Choice certification is a contingency of employment. As per NJ Division of Aging Services, Office of Community Choice Options guidance, an exam score of 80% or higher is required, along with successful completion of the State training modules, and field mentoring component.

Experience in case management or discharge planning Managed care experience Strong crisis intervention skills Education & Licensing Master’s Degree Independent license in NJ LSW or LCSW Anticipated Weekly Hours 40 Time Type Full time Pay Range The typical pay range for this role is: $66,575.00 - $142,576.00 This pay range represents the base hourly rate or base annual full-time salary for all positions in the job grade within which this position falls. The actual base salary offer will depend on a variety of factors including experience, education, geography and other relevant factors. This position is eligible for a CVS Health bonus, commission or short-term incentive program in addition to the base pay range listed above. Our people fuel our future. Requirements

Requires a valid LSW or LCSW license and at least 2 years of behavioral health experience. Candidates must have reliable transportation for travel up to 75% of the time and successfully complete the NJ Choice certification.
