Department: Case Management

Reports To: Director of Case Management/Case Management Supervisor

Position Summary

Mission Community Hospital is seeking a compassionate, organized, and detail-oriented Licensed Vocational Nurse (LVN) to join the Case Management Department. The ideal candidate will have a minimum of one (1) year of Skilled Nursing Facility (SNF) experience and will assist the Case Management team in coordinating patient care, discharge planning, utilization management, and transitions of care to ensure quality outcomes and continuity of services.

Minimum Qualifications

  • Current California Licensed Vocational Nurse (LVN) license in good standing.
  • Minimum of one (1) year of recent Skilled Nursing Facility (SNF) experience required.
  • Current Basic Life Support (BLS) certification.
  • Knowledge of discharge planning, post-acute care services, and care coordination preferred.
  • Strong organizational, communication, and documentation skills.
  • Ability to work collaboratively with physicians, nurses, social workers, and interdisciplinary teams.
  • Proficiency in electronic medical records (EMR).
  • Essential duties and responsibilities
  • Assist the Case Management team with discharge planning and coordination of post-acute care services.
  • Coordinate referrals to Skilled Nursing Facilities (SNFs), home health agencies, rehabilitation facilities, hospice, durable medical equipment (DME) providers, and other community resources.
  • Communicate with insurance companies, referral agencies, and healthcare providers to facilitate timely patient transitions.
  • Review clinical documentation to support appropriate discharge planning and continuity of care.
  • Monitor patient progress and identify potential barriers to discharge.
  • Collaborate with physicians, bedside nurses, therapists, and social workers to ensure safe and timely patient discharges.
  • Maintain accurate and timely documentation in the electronic medical record.
  • Assist with obtaining prior authorizations and coordinating payer requirements as assigned.
  • Educate patients and families regarding available post-discharge services and care options.
  • Participate in interdisciplinary rounds and Case Management meetings.
  • Ensure compliance with hospital policies, regulatory requirements, and patient confidentiality standards (HIPAA).
  • Perform other duties as assigned by the Director of Case Management.
  • Knowledge, skills, and abilities
  • Working knowledge of acute care discharge planning and post-acute care resources.
  • Familiarity with Medicare, Medi-Cal, and commercial insurance requirements is preferred.
  • Strong critical thinking and problem-solving skills.
  • Excellent verbal and written communication skills.
  • Ability to prioritize multiple tasks in a fast-paced acute care environment.
  • Strong customer service and patient advocacy skills.
  • Physical requirements
  • Ability to sit, stand, walk, bend, and move throughout the hospital as needed.
  • Ability to occasionally lift up to 25 pounds.
  • Ability to use computers and office equipment for extended periods.
  • Schedule
  • Full-time position.
  • Monday through Friday with occasional weekend or holiday coverage as operational needs require.
