The Case Manager advocates for clients by resolving external barriers to treatment and managing their accounts and insurance. They are responsible for initiating case management upon intake and facilitating comprehensive discharge planning, including housing and continuing care.

Advocate for client, addressing and resolving any external barriers to treatment Initiate case management for each client upon intake, addressing any pressing needs Oversee client accounts, ensuring insurance is active throughout duration of treatment Provide ongoing case management throughout the duration of treatment Arrange housing, financial aid, legal aid, vocational training etc. as necessary for client post - discharge Facilitate comprehensive discharge planning (continuing care, housing, sober support etc.)
High School diploma or GED preferred Min. 1-year experience in treatment industry preferred If in recovery, a minimum one-year period of sustained sobriety is required Excellent communication, multitasking, and time management skills Strong problem-solving and critical thinking skills Warm, empathetic, respectable personality
