The Behavioral Health Case Manager provides comprehensive support and coordination of care for clients facing mental health challenges. · Coordinate and provide care athat is safe, timely, effective, efficient, and client-centered. · Manage case assignments, draft service plans, and review case progress regularly. · Advocate for clients, assisting them in making informed decisions regarding their clinical status and treatment options. · Collaborate with therapists, clients, and client family members to ensure all client behavioral health needs are met. · Facilitate clients in achieving personal independence and assist in developing daily living skills. · Perform other duties as assigned. · Upholds Documentation and Billing practices associated with CARF, CMS, and as training indicates. Supervision Given: This position does not supervise any staff. Supervision Received: The Behavioral Health Case Manager reports to their assigned Operations Managers.
· Strong interpersonal skills and the ability to communicate effectively with clients, families, and professionals. · Knowledge of behavioral health practices and community resources. · Ability to work independently and as part of a team.
· High school diploma or equivalent is required; · a college degree in Human Services is preferred.
· CPR certification is preferred. · NPI number/Ohio Medicaid ID is required (instructions on how to apply will be provided).
· Must pass all Federal and State background checks according to company policy.
