Franciscan Health Rensselaer 1104 E Grace St Rensselaer, Indiana 47978 The Physical Therapy Assistant I is responsible to the Department Director and supervising Physical Therapist, to provide treatment to the patient prescribed by the physician within the guidelines of the Physical Therapy Practice Act, and hospital and departmental policies and procedures. This position implements the plan of care to restore and enhance performance. This position also performs documentation and quality improvement, as outlined in departmental policies and procedures.

Follow interventions developed for both short- and long-term outcomes. Teach appropriate treatment, procedures and supportive activities to patients and family members. Communicate with patient, Physical Therapist, and Physician to contribute to a collaborative care plan. Communicate changes in patient condition and resolves problem list with Physical Therapist based on care coordination. Attend mandatory and clinical educational offerings. Demonstrate knowledge of home health regulatory standards and policies of agency.
