Promotes safe and cost-effective medication therapy through clinical pharmacy services and the execution of medication initiatives. Responsibilities include medication reconciliation, patient rounding, and managing controlled substances security.

Position overview

Promotes safe, efficacious, and cost-effective medication therapy by participating in the provision and documentation of clinical pharmacy services and execution of medication initiatives.

Maintain security of pharmacy and controlled substances.

Responds to medication related inquiries from other healthcare professionals and patients.

Support hospital auto-substitution policies and other pharmacy dosing programs.

Participate in medication reconciliation patient interviews, medication safety audits, automated dispensing cabinet audits, DEA and diversion monitoring, antibiotic stewardship, opioid stewardship, IV to PO conversion, renal dosing, vancomycin/aminoglycoside dosing, TPN monitoring and management, and assist with additional pharmacy clinical program development.

Participate in daily patient rounding and pharmacy team huddles.

Effectively communicate with providers and provide ongoing discussions regarding safe medication practices.

Reports to Director of Pharmacy.

Education

  • Graduate of a college of Pharmacy
  • Doctor of Pharmacy degree, preferred

Experience

  • One to two years of hospital pharmacy experience preferred.
  • Hospital pharmacy residency, preferred

License/Certifications

• Current Montana licensed Pharmacist

Knowledge/Skills/Abilities

  • Very good drug knowledge base including pharmacokinetics dosing and nutritional support
  • Working computer knowledge in Meditech.
  • Experience working with Automated Dispensing Cabinets.
  • Effective communication skills, verbal and written.
  • Repetitive use of hands and fingers (i.e. use of computer keyboard). Repetitive tasks are often performed with one hand only.
  • normal range of hearing and vision to perform work and communicate.
  • Verbal and written communication.
