Evaluates patients and determines each patient’s need for skilled therapy based on objective and documented criteria.
Conducts evaluations and develops treatment plans according to standardized and/or recommended techniques and procedures.
Follows treatment plans in an objective, measurable, and documented approach maintaining timely and accurate records according to policies, procedures and regulations.
Documents plan of care on evaluation, updates progress notes and provider communication to include but not limited to short and long term goals, and continued need for skilled services when appropriate.
Documents orders and provides clarification orders when appropriate.
Maintains active and supportive communications with the patient, significant others, physicians and other team members concerning progress, problems, home programs, follow-up, and other issues related to the therapeutic process.
Maintains a positive rapport with all staff at all times through professional appearance and actions.
Communicates pertinent information on a timely basis to supervisor and seeks guidance necessary for performance of duties. Responds appropriately to clinical supervision as indicated.
Maintains knowledge of current educational and clinical guidelines, trends, and procedures.
Coordinates and communicates care and discharge plans with appropriate medical staff including but not limited to Physicians, Physical Therapy, Speech Language Pathology, Respiratory Therapy, and Dieticians
