Bringing to each case an understanding of the biopsychosocial concerns of families and the influences of family, community and cultural differences, the Social Worker is responsible for providing high quality and effective case management. The Social Worker acts as a liaison between the client, healthcare providers, and community resources to coordinate comprehensive care and support services and advocates on behalf of clients to ensure they receive the necessary assistance to live as independently as possible.
Candidates must hold a recognized degree in Social Work and be registered as an accredited Social Worker with SASW. Prior experience working in a similar setting is considered an advantage.
