About the role

Provide community health navigation and outreach to individuals with substance use disorders, particularly those experiencing homelessness. Facilitate access to healthcare, Medication Assisted Treatment, and essential community resources while maintaining detailed documentation.

Position Overview

This position serves a dual role, providing community health navigation services while conducting outreach to individuals affected by substance use disorders (SUD), particularly those experiencing homelessness or other barriers to care. This position is dedicated to improving access to healthcare, behavioral health services, treatment, housing, and other essential community resources.

Essential Job Responsibilities

  • Works both in the community and at the Community Resource Center to build trusting relationships with individuals with opioid use disorder and/or substance use disorder to ensure they have the tools to address barriers to seeking care. Provide resources and information to support individuals in accessing and maintaining treatment. Ensure individuals have access to all forms of Medication Assisted Treatment (MAT). Distribute harm reduction materials including but not limited to naloxone and fentanyl testing strips. Observe all rules of confidentiality relating to clinical information and treatment within the agency and when communicating with external agencies or individuals. Maintain records and documentation of interactions, services, and materials provided and report in the assigned reporting mechanism. Actively identify and support linkages to community resources that support the clients’ goals and interests. This will involve a collaborative effort including the client, agency staff, and other relevant community partners. Link clients to appropriate professional resources when needed. Identify barriers to full participation in community resources and develop strategies to overcome identified barriers. Maintain contact with individuals to ensure their ongoing success and to provide re-engagement support if needed. Attend all assigned meetings and required trainings with Ross County Health District, Paint Valley ADAMH, and regularly scheduled monthly meetings including Continuum of Care and Hope Partnership Project. Use computer skills and software to enter and manage program data. Track attendance, participation, and performance data related to peer support activities through Ross County Health District’s Health Navigator of Southeastern Ohio grant.
  • Qualification
  • Requirements: High School diploma or GED required. A Minimum of 2 years’ experience in social services. Individual must have an understanding of and respect for each individual’s path to recovery. Must have a demonstrated commitment to the recovery community. Communicates well both verbally and in writing, creates accurate and punctual reports, delivers presentations, shares information and ideas with others, and has good listening skills. May work in a variety of environments including professional offices, treatment providers, outreach and residential centers, and community events. The employee is required to stand; walk; sit; reach with hands and arms; climb or balance; and stoop, kneel, crouch or crawl and lift up to 40 pounds. Must have a Valid Driver License. Must have a High School Diploma or GED. CDCA or Peer Support certification is preferred. Must pass a Drug Screen and Background Checks.
