Position.
We are seeking a Case Manager to provide fill-in coverage for care coordination services at KSRC Surgery and Inpatient Rehab facility.
This position assists with psychosocial assessments, discharge planning, crisis support, community resource referrals, and communication with patients, families, physicians, and hospital staff.
This role is a great opportunity for a licensed social worker or registered nurse who enjoys patient advocacy, discharge planning, and working collaboratively with a supportive surgical hospital team.
Hours.
This flexible PRN position will work on an as-needed basis but must be available to assume full-time responsibilities to provide operational continuity during manager absences including staff support, patient care coordination, and regulatory compliance.
Experience in healthcare case management, acute care, post-operative care, in-patient rehab and discharge planning preferred.
Education Must have a Master of Social Work or RN license issued by the State of Kansas.
Benefits are not available for PRN staff.
