Provide social work services and counseling to hospice patients and families to address financial, social, and emotional concerns. Collaborate with an interdisciplinary team to develop and maintain individualized plans of care to maximize patient quality of life.
Assist and counsel hospice patients and families with health related financial, social and emotional concerns according to the physician's orders.
Has a Bachelor's degree in social work from an institution accredited by the Council on Social Work Education; or a Bachelor's degree in psychology, sociology, or other field related to social work and is supervised by a Master's prepared Social Worker; and Has 1 year of social work experience in a healthcare setting; or Has a Bachelor's degree from a school of social work accredited by the Council on Social Work Education, is employed by the hospice before December 2, 2008, and is not required to be supervised by a Master's prepared Social Worker .
