Position Summary

The Psychiatric Rehabilitation Counselor provides community-based mental health rehabilitation and support services to adults living with mental illness. Working under the supervision of a Community Support Services LCSW, LPC, RN, or designated supervisor, this role assists consumers in developing independent living skills, improving community functioning, achieving personal recovery goals, and accessing needed resources and services.

The Psychiatric Rehabilitation Counselor plays an integral role on the interdisciplinary treatment team and contributes to the organization’s quality improvement initiatives and overall service excellence.

Essential responsibilities

Provide a minimum of five (5) hours per day of direct, face-to-face psychiatric rehabilitation services in accordance with individualized rehabilitation plans.

Document services provided, consumer progress, observations, and relevant information in a timely and accurate manner.

Assist in the development, implementation, monitoring, and revision of individualized rehabilitation plans.

Serve as a key member of the interdisciplinary team by providing insight into consumers’ mental health status, behaviors, strengths, challenges, and progress toward goals.

Deliver rehabilitative skill-building interventions focused on independent living, socialization, community integration, and recovery.

Conduct functional assessments and provide therapeutic training and on-site consumer supervision as needed.

Support consumers in developing and maintaining life skills necessary for successful community living.

Perform crisis intervention and respond appropriately to behavioral health emergencies.

Coordinate care and communicate effectively with treatment team members, community providers, and support systems to ensure continuity of services.

Establish and maintain collaborative relationships with community agencies, service providers, and vocational resources.

Interview consumers to gather information regarding educational background, employment history, interests, and social needs.

Coordinate training and support services related to vocational development, rehabilitation, and consumer recovery goals.

Facilitate consumer groups and psychoeducational activities.

Provide transportation to appointments, training programs, community resources, and other approved activities as required.

Review diagnostic and assessment information to identify service needs and facilitate referrals and linkages to appropriate resources.

Maintain accurate and current clinical records, treatment documentation, and other required program records.

Participate in agency committees, workgroups, staff meetings, and quality improvement initiatives.

Perform other duties as assigned.

Education & experience

Required

  • Bachelor’s degree from an accredited college or university in a behavioral health or closely related field, including:
  • Social Work
  • Human Services
  • Counseling
  • Psychiatric Rehabilitation
  • Psychology
  • Criminal Justice

Alternative Qualification

• Candidates with a bachelor’s degree outside of the healing professions must possess a minimum of two (2) years of professional experience in a community behavioral healthcare setting.

Licensure, certifications & training

Required

  • Valid driver’s license with an acceptable driving record that meets agency insurance requirements.
  • Successful completion of agency orientation and competency-based adult mental health rehabilitation training upon hire.

Preferred

• Psychiatric Rehabilitation Certification (CPRP) or equivalent psychiatric rehabilitation credential.

Knowledge, skills & abilities

Knowledge of psychiatric rehabilitation principles, recovery-oriented care, and community mental health services.

Ability to work effectively with individuals who have significant mental health challenges and histories of psychiatric hospitalization.

Strong interpersonal, communication, and relationship-building skills.

Ability to assess consumer needs and coordinate appropriate services and supports.

Crisis intervention and de-escalation skills.

Ability to maintain clinical documentation in accordance with organizational and regulatory standards.

Proficiency with electronic health records and other computer-based documentation systems.

Ability to work independently and as part of a multidisciplinary team.

Effective organizational and time-management skills.

Ability to read, write, speak, and communicate effectively in English.

Ability to successfully complete all required training and professional development activities.

Working conditions

This position involves community-based service delivery and may require travel to consumer homes, community agencies, medical appointments, and other service locations. The role requires direct interaction with adults experiencing behavioral health challenges and may involve responding to crisis situations.

Requirements

Requires a bachelor's degree in a behavioral health field or a bachelor's degree in another field with at least two years of professional experience in community behavioral healthcare. A valid driver's license and completion of agency-specific mental health training are mandatory.
