  • What You'll Do
  • Perform occupational therapy evaluations in patients' homes.
  • Develop individualized treatment plans that improve safety, independence, and quality of life.
  • Provide interventions to enhance activities of daily living (ADLs), mobility, home safety, and fall prevention.
  • Recommend adaptive equipment and home modifications.
  • Collaborate with physicians, nurses, caregivers, and the interdisciplinary care team.
  • Complete timely, accurate documentation in accordance with home health standards.

Qualifications

  • Current Occupational Therapist license in the state of practice.
  • Graduate of an accredited Occupational Therapy program.
  • Valid driver's license and reliable transportation.
  • Home health experience is a plus, but new home health clinicians are welcome to apply.
