Responsibilities

Utilization of a person-centered planning process to assist individuals and their families in developing an individualized service plan that integrates their goals and priorities Assist resident with activities of daily living including grooming, dressing, bathing; may also assist feeding Assist resident with keeping bedroom tidy and safe; assist resident with laundry tasks Perform kitchen duties including meal preparations, special diet preparations and cleaning Provide information, support, and training to assist consumers in achieving stated independent living goals in their individual service plan and/or make referrals to appropriate resources and agencies Provide enhanced information and assistance by working with the consumer to identify the individual's long- and short-term needs, identifying resources to meet those needs, providing referrals to identified resources, and, where appropriate, providing warm hand offs, and following up to ensure a consumer's needs have been met Plans, escorts, participates in, and supervises resident during community and group activities, including recreational, leisure, vocational, and educational pursuits, to build skills and develop appropriate coping mechanism Implemented and followed Individual Service Plans, Behavior Management Plans, Independent Living Plans, and Personal Support Plans Observe, report, and record data concerning physical, emotional, and behavioral states May assist, as assigned, in medication distribution programs Transport or escort consumers to and from recreational activities, appointments, and other destinations Utilizes H.E.L.P. (including therapeutic holds) when it is determined that a resident is a danger to self or others; also performs basic First Aid and/or CPR when needed Attends and participates in mandatory training, in-services, and staff meetings Utilizes and accurately records data using AWARE's Electronic Medical Records System as assigned, outlined, and trained. Provides assistance to direct supervisor in scheduling and administrative tasks.

Requirements

Talents, skills, and abilities

At least three (3) years of experience is required and/or an associate degree in a Human Services field, or a minimum of a bachelor’s degree in any field, and a minimum of two years of appropriate and related experience in Human Services. Ability to, under AWARE’s Unconditional Care Philosophy, implement individual support plans with the knowledge of behavioral techniques, interventions, skill-building techniques, and understanding of the emotional and behavioral needs of individuals within the residence. Requires computer literacy skills, including the ability to navigate, record, and overall basic computer skills needed for performance-based software as well as an electronic medical records system. The Residential Specialist must have interpersonal, oral, and written communication skills to work as a team member, make observations, report them accurately, learn resident care methods, and follow oral and written instructions.
