Position Summary

The Home Health Aide (HHA) provides personal care and supportive services to patients in their homes under the supervision of a Registered Nurse (RN) and in accordance with the patient’s plan of care, physician orders, agency policies, and applicable state and federal regulations.

This role focuses on assisting patients with daily living activities, supporting safety and comfort, and reporting changes in patient condition to the supervising clinical team.

Reporting Structure

This position reports to the Registered Nurse (RN) or designated clinical supervisor. The Home Health Aide functions as part of the interdisciplinary care team and provides care strictly within the established plan of care under RN supervision.

Scope of Practice

The Home Health Aide performs non-skilled personal care services within the scope of Florida HHA certification, Medicare Conditions of Participation, and agency policies. This role does not perform skilled nursing procedures, administer medications, or make clinical decisions.

Essential Job Functions

  • 1. Personal Care Assistance
  • Assist patients with activities of daily living (ADLs), including bathing, dressing, grooming, toileting, and hygiene
  • Provide assistancewith mobility, transfers, positioning, and ambulation
  • Assist with feeding and hydration as directed in the care plan
  • Maintain a safe, clean, and comfortable environment for patients
  • 2. Patient Support & Safety
  • Provide companionship and emotional support to patients and families
  • Observe and report changes in patient condition, behavior, or environment to the supervising RN
  • Support patient safety and fall prevention strategies
  • Reinforce care plan instructions provided by clinical staff
  • 3. Basic Care Activities
  • Assist with prescribed exercises under direction of clinical staff
  • Assist with skin care and basic preventive care measures as outlined in the care plan
  • Support patient independence and dignity in all aspects of care
  • 4. Documentation & Communication
  • Document care provided accurately and in a timely manner according to agency standards
  • Reportpatient concerns, changes in condition, or safety issues to the supervising RN immediately
  • Maintain confidentiality of all patient information in accordance with HIPAA
  • 5. Team Collaboration
  • Work closely with nurses, therapists, and other care team members to support continuity of care
  • Participatein required in-services, training, and staff meetings
  • Follow infection control, safety, and compliance protocolsat all times

Qualifications

  • High school diploma or equivalent required. Completion of a state-approved Home Health Aide (HHA) or Certified Nursing Assistant (CNA) training program required. Current HHA certification (or CNA certification if applicable under state regulations) required.
  • Minimum of 1 year of patient care experience preferred
  • Home health, hospice, long-term care, or hospital experience preferred
  • Current CPR certification required
  • Valid driver’s license, reliable transportation, and auto insurance
  • Ability to pass background screening and all compliance requirements

  • Ability to work in home environments with varying conditions and accessibility

Work is performed in patient homes and community settings with exposure to variable environments, including pets, smoke, infectious diseases, and weather-related travel conditions.

Requirements

Requires a high school diploma and completion of a state-approved HHA or CNA training program with current certification. Must possess a valid driver's license, reliable transportation, and current CPR certification.
