As the Behavioral Health Advocate, you will
1. Under direction from the Behavioral Health Supervisor and Women’s Health Director, provide care coordination services to patients who are most at risk for health deterioration, Social Determinates of Health, sentinel events and/or poor outcomes. 2. Assist the Medical Oncology team and the Behavioral Health Provider in maintaining a central role in the coordination and management of care of vulnerable patients and that patients receive comprehensive care across multiple health settings and providers; to ensure continuity and coordination of care across transitions of care for all patients, with special attention to patients with complex medical, behavioral and social needs.
Associate or bachelor's Degree in related field is preferred.
Significant experience in the behavioral health and/or substance abuse field will be considered, in place of a degree.
Current BLS through the American Heart Association (AHA) or obtained within 3 months of hire.
CAC I or higher preferred or willing to obtain.
