We are seeking a Nephrology Nurse Practitioner who is passionate about making a measurable impact—not just treating disease but transforming lives.

Position Overview

The Nephrology NP plays a critical role in delivering high-quality, coordinated kidney care across outpatient clinics, dialysis units, and hospital settings. This position supports RMA’s mission to improve patient outcomes, reduce barriers to care, and enhance the referral experience for providers.

Key Responsibilities

  • Deliver High-Quality Clinical Care
  • Evaluate and manage patients with CKD, ESRD, hypertension, electrolyte disorders, nephrolithiasis, and other renal-related conditions
  • Develop proactive care plans focused on slowing disease progression
  • Optimize medication management and patient outcomes
  • Provide timely follow-up and continuity of care
  • Support smoother transitions between clinic, hospital, and dialysis settings
  • Patient Education & Advocacy
  • Educate patients and families on kidney disease, treatment options, and lifestyle modifications
  • Provide counseling on dialysis modalities and transplant readiness
  • Support patients’ emotional and psychosocial needs
  • Support Dialysis & Advanced Therapies
  • Conduct dialysis rounds (in-center and home modalities)
  • Manage vascular access issues and dialysis-related complications
  • Educate patients on home dialysis and transplant options
  • Develop and implement individualized treatment care plans
  • Care Coordination
  • Collaborate with nephrologists, nurses, dietitians, and social workers
  • Coordinate care for patients with multiple comorbidities (e.g., diabetes, hypertension)
  • Facilitate referrals and ensure continuity of care

Qualifications

  • Required
  • Master’s or Doctorate in Nursing (MSN or DNP)
  • Current licensure as a Nurse Practitioner in the State of New Mexico (or eligibility)
  • National board certification (AANP, ANCC, or equivalent)
  • DEA license and prescriptive authority
