About the role

The Case Manager supports individuals recovering from addiction and mental illness by conducting assessments and developing personalized recovery plans. They coordinate with healthcare providers and community resources while providing emotional support and life skills education.

The primary focus of a Case Manager is to support individuals in their recovery journey from addiction, mental illness, or both.

Job Responsibilities

  • Conduct initial and ongoing assessments to evaluate clients' mental health, substance use, strengths, and needs.
  • Develop personalized recovery plans in collaboration with clients, outlining specific goals and interventions.
  • Coordinate with healthcare providers, social services agencies, and community resources to ensure comprehensive support for clients.
  • Advocate for clients' rights and preferences within the healthcare and social service systems.
  • Provide emotional support and encouragement to clients throughout their recovery process.
  • Educate clients about mental health and addiction issues, treatment options, coping strategies, and relapse prevention techniques.
  • Teach life skills such as budgeting, time management, and communication to help clients achieve greater independence.
  • Document all interactions, services provided, and outcomes accurately and timely.
  • Work closely with multidisciplinary teams including psychiatrists, therapists, nurses, social workers, and case managers to ensure integrated care.

1. Ethical and Legal Compliance

  • Adhere to ethical standards and legal regulations in all aspects of service delivery, including confidentiality and privacy laws. Knowledgeable in 42/CFR Part 2 and HIPAA.
  • Maintain accurate records and ensure compliance with documentation requirements.

Qualifications

  • Previous experience in case management, mental health, substance abuse treatment, or related fields is preferred. Experience working with individuals in recovery or in crisis situations is beneficial.
  • Strong interpersonal and communication skills are essential. Organizational skills, problem-solving abilities, and the ability to work collaboratively in a team environment are also important.
