Avalon View Health and Wellness is seeking an experienced and compassionate Wound Care and Infection Prevention Nurse to join our clinical team in Liberty, Missouri.
This full-time position is responsible for assessing, treating, monitoring, and documenting acute and chronic wounds while helping coordinate the community’s Infection Prevention and Control Program. The nurse will work closely with residents, families, nursing personnel, physicians, and other members of the interdisciplinary care team to support healing, reduce infection risks, and promote positive clinical outcomes.
Avalon View works with a wound care physician who visits the community weekly, providing an opportunity for ongoing clinical collaboration and coordinated treatment planning.

The Wound Care and Infection Prevention Nurse is responsible for coordinating resident wound care services and supporting facility-wide infection prevention practices.
This position conducts resident assessments, develops and updates individualized treatment recommendations, performs wound care procedures within the nurse’s authorized scope of practice, monitors healing, and communicates changes to physicians and other members of the clinical team.
The nurse also helps develop, implement, monitor, and evaluate infection prevention practices throughout the community in accordance with applicable federal and Missouri requirements, evidence-based guidance, and facility policies.
Wound care responsibilities
Conduct comprehensive skin and wound assessments to identify wound type, location, measurements, stage, severity, drainage, surrounding skin condition, and signs of infection
Develop and update individualized wound care recommendations based on resident needs, physician orders, and evidence-based practices
Perform dressing changes, topical treatments, and other wound care procedures within the nurse’s authorized scope of practice
Complete debridement only when authorized by licensure, training, physician orders, and facility policy
Monitor residents for changes in wound condition, pain, skin integrity, and overall health status
Promptly communicate significant changes or concerns to the attending physician, Director of Nursing, responsible party, and other appropriate members of the care team
Collaborate with the visiting wound care physician and interdisciplinary team to coordinate treatment and support positive outcomes
Monitor pressure injury prevention interventions, including positioning, pressure redistribution, nutrition, hydration, and incontinence-related skin care
Review treatment supplies and help ensure necessary wound care products are available
Educate residents, families, and nursing personnel about wound care, skin protection, pressure injury prevention, and signs of infection
Maintain accurate and timely documentation of assessments, treatments, measurements, progress, physician communication, and resident responses
Participate in care plan meetings, quality assurance activities, clinical reviews, and facility surveys as assigned
Monitor trends and outcomes related to wounds, skin conditions, and pressure injuries

Candidates must be a graduate of an approved nursing program with an active Missouri RN or LPN license and BLS/CPR certification. A certification in Infection Prevention and Control is required, while wound care certification and long-term care experience are strongly preferred.
