The Case Navigator is responsible for providing comprehensive case management services to individuals living with HIV/AIDS. This role involves assisting clients in accessing housing services, healthcare, and other essential resources. The Case Navigator works closely with clients to develop and implement individualized service plans, advocating for their needs and supporting them in achieving housing stability and improved overall well-being. PRINCIPAL DUTIES Conduct comprehensive assessments utilizing the EMPath case management model to identify clients' strengths, challenges, and goals for achieving housing stability. Collaborate with clients to develop individualized service plans that address housing challenges and support overall health and well-being. Identify and assess resources and services needed to achieve clients' goals. Carres a caseload of families and individuals, which may include home-based case management. Assist clients in securing safe and stable housing, coordinating with landlords, property managers, and housing authorities as needed. Provide guidance on rental assistance programs, subsidies, and other housing-related resources. Advocate for clients facing discrimination or challenges related to housing. Connect clients with community resources, including employment services, legal assistance, food assistance, and other support services. Stay informed about available resources and maintain a comprehensive resource directory. Assist clients in navigating bureaucratic processes and accessing entitlement programs. Provide crisis intervention and support during emergencies or challenging situations. Collaborate with clients to develop strategies for overcoming crises and preventing future challenges. Maintain regular contact with clients to monitor their well-being and address emerging needs. Maintains precise and accurate documentation of case management services, including client files and entries into the program databases. Maintain accurate and up-to-date client records, documenting all interactions and services provided. Prepare and submit reports on client progress, outcomes, and program activities as required. Ensure compliance with data management protocols and confidentiality standards. Demonstrate cultural competence and sensitivity in working with a diverse population, respecting individual identities, backgrounds, and experiences. Stay informed about the unique needs and challenges faced by individuals living with HIV/AIDS from different communities. Performs other duties as assigned. QualificationsMINIMUM QUALIFICATIONS Associate Degree in psychology or related field. One (1) year of experience in case management Certified in National Standards for the Physical Inspection of Real Estate (NSPIRE) or able to obtain within 6 months of employment. Valid Texas Driver’s License SKILLS REQUIREMENTS Flexible and effective team player, able to adapt to changes and a busy workplace environment. Strong interpersonal and communication skills. Knowledge of housing resources, entitlement programs, and community services. Ability to work independently and as part of a collaborative team. Empathy, resilience, and a commitment to advocating for the needs of clients. Catholic Charities is an equal opportunity employer. Requirements

Requires an Associate Degree in psychology or a related field and at least one year of case management experience. Must possess a valid Texas Driver's License and be able to obtain NSPIRE certification within six months.
