  • Patients: Conduct comprehensive evaluations to identify physical, cognitive, communication, and/or swallowing needs based on discipline-specific scope of practice.
  • Develop &

  • Plans: Create and carry out individualized treatment plans and rehabilitation programs to address functional deficits and promote independence.
  • Provide
  • Therapeutic
  • Interventions: Deliver skilled, hands-on therapy and therapeutic activities, including exercises, manual techniques, adaptive strategies, assistive device training, and communication or swallowing interventions as appropriate.

  • Staff: Provide education such as but not limited to pain management, fall prevention, home safety, coping strategies, communication techniques, and strategies to improve daily functioning and quality of life.

  • Care: Track patient outcomes, document evaluations and progress, and modify treatment plans based on patient response and clinical judgment.

  • Qualifications: Must be a graduate from an Accredited Therapy Program and have a Bachelor's Degree Must possess current licensure for MN, ND or IA Must possess strong knowledge regarding state, federal, and local regulations as they pertain to Home Health care. Must be capable of performing the essential functions of the job, with or without reasonable accommodations. Must be able to communicate in English, both verbally and in writing and possess sufficient communication skills to perform the tasks required.

Requirements

Candidates must be graduates of an accredited therapy program with a bachelor's degree and hold current licensure for MN, ND, or IA. Strong knowledge of state and federal Home Health care regulations is required.
