Hospice Medical Social Worker (MSW) Bluebird Home Health and Hospice is seeking a compassionate and qualified Medical Social Worker (MSW) to join our hospice care team. This role provides psychosocial assessments, counseling, resource coordination, and support to patients and families facing terminal illness. The MSW works closely with the interdisciplinary team to help ensure each patient receives coordinated, respectful, and individualized care.

Qualifications

Master of Social Work (MSW) degree required. Licensed in the State of Idaho. Valid driver’s license and reliable transportation required. Home health and/or hospice experience preferred. Strong communication, documentation, and interpersonal skills. Ability to work collaboratively with patients, families, caregivers, community agencies, and interdisciplinary team members.

Responsibilities

Assess the psychosocial status of patients and families/caregivers related to terminal illness, home environment, and care needs. Communicate assessment findings to the registered nurse and interdisciplinary care team. Conduct social evaluations and develop appropriate interventions based on patient and family needs. Provide counseling and emotional support related to stress, coping difficulties, grief, health concerns, and end-of-life issues. Maintain accurate and timely clinical documentation for all patients referred to social work services. Provide information, referrals, and assistance regarding practical, financial, environmental, and community resource needs. Serve as a liaison between patients, families/caregivers, community agencies, and the hospice team. Maintain collaborative working relationships with organization personnel to support quality patient care. Develop and maintain relationships with public and private agencies that may serve as patient and staff resources. Assist the volunteer coordinator in identifying patient and family needs for volunteer services. Participate in individualized plan of care development and regularly scheduled interdisciplinary group meetings. Follow organizational documentation standards, care procedures, and expectations for professional conduct. Assist physicians and team members in understanding significant social and emotional factors related to health problems, death, and dying. Participate in quality assessment and performance improvement activities. Perform other duties as delegated by the Clinical Supervisor. Work Environment This position includes providing services in the patient’s identified residence and assisting with alternative planning when the home environment is not safe or appropriate.
