About the Role

Clarvida is seeking a Treatment Foster Care Case Manager to support youth with significant behavioral, emotional, or medical needs in a family-based treatment setting. This role is an essential member of the Treatment Team, coordinating behavioral health and community-based services to help youth stabilize, achieve treatment goals, maintain placement, and work toward permanency with their families. As a Case Manager, you will collaborate closely with foster parents, clinicians, the Idaho Department of Health & Welfare, and birth families to develop individualized treatment plans and ensure consistent progress. This position offers a flexible schedule, supportive team environment, and strong opportunities for professional growth.

Responsibilities

  • ● Develop individualized treatment goals and service plans for children and youth in Treatment Foster Care ● Coordinate behavioral health, educational, and community-based services ● Monitor treatment progress, update goals, and support readiness for discharge ● Provide regular communication and support to foster parents ● Partner with birth families as appropriate to support permanency planning ● Maintain accurate documentation and timely progress notes ● Participate in clinical supervision, team meetings, and ongoing training Required Qualifications ● Bachelor’s degree (Human Services field preferred): Social Work, Psychology, Sociology, Counseling, Family Relations, Nursing, OT, PT, TR, SLP, Behavioral Sciences, Art Therapy, etc. ● Ability to pass background checks, including Enhanced Fingerprinting with Idaho DHW ● Valid driver’s license, reliable transportation, and proof of auto insurance ● Experience with EHR systems is a plus Compensation & Benefits ● Flexible schedule you create ● Hybrid and remote work ability ● Productivity incentives ● Part-time caseload that can build to full-time ● Multiple locations available ● Stability and growth working with a national agency Full-Time Employees ● Paid vacation days that increase with tenure ● Separate sick leave that rolls over annually ● Up to 10 paid holidays
