The RN Case Manager is responsible for creating and managing personalized care plans for individual patients. This includes coordinating with colleagues, administering medications, and maintaining accurate patient records.

Coordinate with other colleagues to assess, plan, implement, or evaluate patient care plans Administer medications and monitor patients for side effects or reactions Document any changes in patient conditions and report on changes Maintain accurate records of patient care and discuss observations with the supervisor Must be available Mondays, Wednesdays, and Fridays. Occasional weekend and holiday on call.
Currently licensed Registered Nurse (RN) in state of employment Previous experience as a Registered Nurse is preferred Excellent interpersonal and client care skills Valid driver’s license First aid/CPR certification required
