Responsibilities

  • Clinical Care The Advanced Practice Provider (APP), either a Nurse Practitioner (NP) or Physician Assistant (PA), serves as an integral member of the interdisciplinary care team, providing comprehensive assessment, triage, diagnosis, treatment, and ongoing management of patients across inpatient, outpatient, and ambulatory settings.
  • In collaboration with supervising physicians and in accordance with state regulations, hospital policies, clinical protocols, and professional practice standards, the APP: Obtains comprehensive medical histories and performs physical examinations.
  • Orders, interprets, and reviews diagnostic, laboratory, and radiologic studies.
  • Develops, implements, and manages evidence-based treatment plans.
  • Prescribes medications and therapeutic interventions within scope of practice and regulatory guidelines.
  • Performs specialty-specific procedures and assists in operative cases when applicable.
  • Provides autonomous, team-based care for designated patient populations, as appropriate.
  • Responds to emergent clinical situations and supports departmental and organizational patient care initiatives.
  • Collaborates with physicians, nurses, and allied health professionals to ensure high-quality, patient-centered care.
  • Patient Education & Psychosocial Support The APP plays a critical role in educating patients and families and supporting them throughout their healthcare journey by: Providing comprehensive education regarding diagnoses, treatment plans, medications, potential side effects, and discharge instructions.
  • Facilitating informed decision-making and promoting health literacy.
  • Identifying and connecting patients and families with appropriate support services and community resources.
  • Delivering compassionate psychosocial support to patients, family members, and caregivers as part of a multidisciplinary team.
  • Information Management The APP is responsible for maintaining accurate, timely, and complete clinical documentation while ensuring compliance with organizational and regulatory standards.
  • Key responsibilities include: Documenting patient encounters, assessments, treatment plans, and clinical outcomes within the electronic medical record.
  • Ensuring compliance with all hospital, accreditation, and regulatory documentation requirements.
  • Safeguarding confidential patient and organizational information across all media formats, including electronic, written, verbal, and digital communications.
  • Quality & Regulatory Compliance The APP demonstrates a strong commitment to clinical excellence, patient safety, and continuous quality improvement by: Upholding the highest standards of professionalism, ethics, integrity, and compassionate care.
  • Ensuring compliance with all applicable healthcare regulations, accreditation standards, policies, and procedures.
  • Participating in quality improvement initiatives, performance improvement projects, and patient safety programs.
  • Identifying and reporting safety events, near misses, and opportunities for system improvement.
  • Professional Development & Leadership The APP is expected to engage in lifelong learning and contribute to the advancement of clinical practice and organizational excellence through: Maintaining licensure, certifications, credentialing requirements, and continuing education standards.
  • Participating in professional development activities that enhance clinical expertise and competency.
  • Serving as a clinical resource and mentor for new providers, house staff, and interdisciplinary team members.
  • Providing clinical education and preceptorship for NP/PA students and other healthcare learners.
  • Collaborating on research initiatives, quality improvement projects, protocol development, and evidence-based practice initiatives.
  • Contributing to a culture of high reliability, accountability, collaboration, and continuous improvement This position offers an opportunity to practice at the top of licensure while working in a collaborative environment dedicated to exceptional patient outcomes, education, innovation, and professional growth.

Qualifications

  • Education Graduate of an accredited Nurse Practitioner (NP) program or an accredited Physician Assistant (PA) program.
  • Master's degree or higher required, consistent with professional licensure and certification requirements.
  • Experience Prior clinical experience as a Nurse Practitioner or Physician Assistant within the applicable specialty or service line is preferred but not required.
  • Demonstrated ability to provide high-quality, patient-centered care within a collaborative healthcare environment.
  • Advanced Practice Registered Nurses (APRNs), Physician Assistants (PAs), Certified Registered Nurse Anesthetists (CRNAs), and Certified Nurse Midwives (CNMs) must be appointed to the Affiliated Medical Staff of a Yale New Haven Health System (YNHHS) hospital and successfully complete the Medical Staff credentialing and privileging process.
  • Candidates must provide evidence of current clinical competency, including professional references from physicians and/or advanced practice providers as part of the credentialing process.
  • Licensure & Certifications For Physician Assistant Candidates: Current certification through the National Commission on Certification of Physician Assistants (NCCPA).
  • Active Connecticut Physician Assistant License or eligibility for Connecticut licensure.
  • Current DEA registration (if applicable to position).
  • Current Basic Life Support (BLS) and/or Advanced Cardiovascular Life Support (ACLS) certification, as required by the position.
  • Pediatric positions may require: Pediatric Advanced Life Support (PALS) certification.
  • Neonatal Resuscitation Program (NRP) certification, when applicable.
  • For Nurse Practitioner Candidates: Active Connecticut Registered Nurse (RN) License.
  • Active Connecticut Advanced Practice Registered Nurse (APRN) License.
  • National board certification in the applicable specialty.
  • Current DEA registration (if applicable to position).
  • Current Basic Life Support (BLS) and/or Advanced Cardiovascular Life Support (ACLS) certification, as required by the position.
  • Additional Requirements Ability to obtain and maintain medical staff privileges at affiliated YNHHS hospitals.
  • Additional certifications, licenses, continuing education, and specialty training may be required based on clinical assignment, department needs, and regulatory requirements.
  • Commitment to ongoing professional development, evidence-based practice, and high-quality patient care.
