The RN Case Manager provides direct skilled nursing care and health education to patients and their families in accordance with physician orders. They are responsible for assessing patient responses to care, documenting services, and collaborating with a multidisciplinary team.
Role and Responsibilities
Administer direct skilled nursing care to patients, in accordance with physician’s orders.
Essential Functions
Provide hands-on, skilled nursing care in accordance with physician’s orders.
Provide health education to the patient and family.
Assess patient response to the plan of care interventions and communicate changes to the case manager.
Collaborate and share findings with the multidisciplinary care team.
Document those services rendered to the patient, note changes in the patient’s condition and/or family and home situations.
Document communication with case manager and physician.
Meet visit productivity requirements and conduct other home visits (SOC/ROC, recertifications, discharges) as requested by clinical manager.
Other duties as assigned.

The RN Procedure Nurse is primarily in a non-office setting and may be exposed to outdoor conditions.
